Glycaemic Load n.

Unlike glycaemic index, which reflects carbohydrate quality alone, glycaemic load incorporates portion size, making it the more accurate predictor of real-world glucose and insulin demand.

The definition

Glycaemic Load is a dietary metric that quantifies the total blood-glucose impact of a food serving, calculated by multiplying its glycaemic index by the available carbohydrate content in grams and dividing by 100. A score of 10 or below is low, 11 to 19 is medium, and 20 or above is high, with lower loads producing smaller postprandial glucose swings.

The mechanism

Glycaemic index was defined in 1981 by Jenkins and colleagues as the postprandial glucose response produced by a test food, expressed as a percentage of the response to an equal-carbohydrate portion of pure glucose 1. Glycaemic load extends this by multiplying glycaemic index by the available carbohydrate content of a serving, then dividing by 100; a result of 10 or below is a low glycaemic load, 11 to 19 is medium, and 20 or above is high 2. Where glycaemic index measures only carbohydrate quality, the rate at which a food's carbohydrate raises blood glucose, glycaemic load adds portion size, producing a truer estimate of the total glucose delivered to the bloodstream after a meal 2.

The distinction matters because a food can carry a high glycaemic index yet a low glycaemic load: watermelon has a glycaemic index near 76 but a glycaemic load of only 4 to 6 per 120-gram serving, since a typical portion contains very little available carbohydrate by weight 2. Repeated high-glycaemic-load eating raises postprandial insulin demand; sustained beta-cell responses of this kind may, over time, blunt insulin sensitivity 3.

Glycaemic load also shapes short-term cognitive fuel supply. A meta-analysis of 15 controlled trials found episodic memory performed significantly better after a low-glycaemic-load breakfast than after a high-glycaemic-load breakfast, from two hours post-consumption onwards, a pattern consistent with reactive hypoglycaemia 4.

In practice

The clearest illustration is a simple breakfast swap that keeps the same rough amount of food on the plate but changes the type of carbohydrate.

Worked example

A bowl of porridge topped with nuts and berries carries a glycaemic load of around 10, while two slices of white toast with jam comes in near 25, despite similar serving sizes. The lower-load option sustains blood glucose for a longer window after the meal, supporting steadier energy through the two to three hours of peak morning mental demand. The higher-load option produces a faster rise and an earlier decline.

Same plate, same rough serving size, but very different demands placed on insulin and very different windows of steady energy.

Why it matters

In a multinational cohort of 127,594 adults from 20 countries followed for a median of 11.8 years, those with the highest dietary glycaemic load had a 21 per cent higher risk of developing type 2 diabetes than those with the lowest 5. That risk was stronger among people with a higher BMI, suggesting habitual high-glycaemic-load intake compounds the metabolic burden of excess adiposity rather than acting independently of it 5.

A separate cohort of 137,851 participants across five continents, followed for a median of 9.5 years, found a high-glycaemic-index diet raised the risk of major cardiovascular events by 51 per cent among people with existing cardiovascular disease and by 21 per cent among those without 3. The two 21 per cent figures come from separate studies measuring different outcomes, diabetes risk and cardiovascular risk, and should not be read as the same result.

What is the difference between glycaemic index and glycaemic load?

Glycaemic index measures only how quickly a food's carbohydrate raises your blood glucose relative to pure glucose. Glycaemic load builds on that by factoring in how much carbohydrate an actual serving contains, so a food with a high index but a small carbohydrate portion, such as watermelon, can still carry a low load.

What are low glycaemic load foods?

Foods with a glycaemic load of 10 or below are classed as low, and typically include legumes, intact whole grains, and non-starchy vegetables. Swapping white rice, white bread, and sugary drinks for these alternatives is the main way you can reduce the overall glycaemic load of your diet without cutting out entire food groups.

Does a high glycaemic load diet cause diabetes?

A high glycaemic load diet is linked to greater risk rather than proven to directly cause the disease. In a cohort of over 127,000 adults followed for nearly 12 years, people in the highest quintile of dietary glycaemic load had a 21 per cent higher risk of developing type 2 diabetes than those in the lowest quintile.

How do I calculate the glycaemic load of a meal?

Multiply the food's glycaemic index by the grams of available carbohydrate in your actual serving, then divide by 100. A result of 10 or below counts as a low glycaemic load, 11 to 19 as medium, and 20 or above as high, so portion size changes the answer as much as the food choice does.
